1. If the offense is a felony, and it has been more than one year since I have been released from all sanctions, including incarceration in a state or local facility or any term of supervision, imposed for that offense.
2. If the offense is a misdemeanor, and it has been more than six months since the I have been released from all sanctions, including incarceration in a state or local facility or any term of supervision, imposed for that offense.
3. Did you earn a Certificate of Achievement and Employability (CAE) while incarcerated with the Ohio Department of Rehabilitation and Correction (DRC)?
4. Did you earn a Certificate of Achievement and Employability (CAE) while under supervision with the Adult Parole Authority (APA)?
5. Did you receive ninety days of credit toward your stated prison term while incarcerated with DRC?
6. Did you receive a ten percent reduction of your stated prison term while incarcerated with DRC?
7. If you were released from incarceration in a local facility, do you have a letter of recommendation of the sheriff or his designee?
8. If the petitioner served a DRC prison term or on Adult Parole Authority supervision, did the petitioner receive a Certificate of Achievement and Employability (CAE)?
9. If the petitioner served a DRC prison term, did the petitioner receive ninety days of credit toward satisfaction of the person’s stated prison term?
10. If the petitioner served a DRC prison term, did the petitioner receive a ten percent reduction of the person’s stated prison term under division (A)(2) of section 2967.193 of the Ohio Revised Code?
